Disability Navigators LLC is a DDD and Medicaid approved Support Coordination agency serving all counties in NJ. Started by parents who have lived the intellectual and developmental disability journey with their own children, we are dedicated to providing the most responsive, and professional Support Coordination Services in the region.

Driven by the belief in the inherent worth of every person with intellectual and developmental disabilities, we partner with individuals, families, caregivers, and providers to deliver person-centered, responsive, consistent and quality-assured support coordination services. We pledge to treat every individual like our own family; with compassion and attention to detail to ensure that each client has the fulfilling and happy life that they desire. Though we are a newer agency, we are authorized by DDD to approve our own plans, and we have over 25 years of experience across our talented and caring staff.

Eligibility	We are able to work with individuals who are approved for DDD services. Typically, these are adults age 21 and above who have intellectual and developmental disabilities, though in some cases, we are able to work with individuals age 18-21.
