Call Central Intake and connect to free or affordable resources to help you and your family meet life's challenges. The purpose of Central Intake is to increase screening, referrals and/or linkage of pregnant women and women of child bearing age to needed services within the county.
Service connections include but are not limited to:
- Pregnant and New Mom Home Visitation
- Pregnancy Testing
- Childcare
- Family Counseling
- Health Care Services
- Domestic Violence
- Substance Abust Treatment
- Food/Nutrition Assistance
Central Intake will aid in accessing the most appropriate services in an efficient manner through referrals and follow-up. It will also coordinate with outreach to locate women/families without telephones to achieve contact for an initial assessment and medical care preconception.
As part of the Partnership for Maternal and Child Health of Northern New Jersey (PMCHNNJ), and funded by the New Jersey Department of Health and Senior Services, the Improving Pregnancy Outcomes initiative addresses the medical, social, and mental health of the community through the use of Community Health Workers (CHW’s) and a Central Intake system. Together, they are designed to have a significant and positive impact on the overall well-being of the county’s residents.
